本标准规定了医疗卫生用品辐射灭菌和消毒的术语和定义、辐射灭菌和消毒要求、产品辐射处理要求、微生物监测方法和要求、辐射产品的放行要求和辐射后的管理要求。
本标准适用于所有开展辐射灭菌和消毒的单位。

术语和定义
下列术语和定义适用于本文件。
3.1:无菌保证水平 Sterilizationassurancelevel;SAL
灭菌后产品上存在单个活微生物的概率,通常表示为10-n。
3.2:D 值 Dvalue
在设定的暴露条件下,杀灭特定试验微生物总数的90%所需的辐射吸收剂量。
3.3:生物负载 bioburden
一个产品或一件包装上存在的活的微生物总数。
3.4:生物指示物 biologicalindicator
对特定灭菌或消毒程序有确定的抗力,可供消毒灭菌效果监测使用的微生物检验器材。
3.5:吸收剂量 absorbeddose
单位质量的物质所吸收的能量的量值。
注: 吸收剂量单位是戈瑞(Gy),1Gy=1J/kg。
3.6:吸收剂量的不均匀度 unevenress
辐射产品箱中,不同部位测得的最大吸收剂量(D max)除以最小吸收剂量(D min)之商,见式(1):
3.7:剂量计 dosimeter
剂量仪,一种能在待定时间内测量所接受的核辐射剂量的仪器。
3.8:工作剂量计 workingdosemeter
经标准剂量计校准过的,用来标定辐射场剂量率与测定产品吸收剂量,进行常规剂量监测的剂量计。
3.9:灭菌剂量 sterilizationdose
为达到特定的灭菌要求所需的最小剂量。
辐射灭菌和消毒要求
4.1 在辐射灭菌前医疗卫生用品的要求
4.1.1 医疗卫生用品及包装材料应是耐辐射灭菌剂量的材料。
4.1.2 医疗卫生用品的初始污染菌应进行检测,保证使用辐射灭菌的有效性。
4.1.3 对于可能影响产品质量的工艺、生产环境等方面出现的异常情况应有纠正措施。
4.1.4 对于可能影响产品质量的工艺、生产环境等方面潜在的危害应采取相应的预防措施。
4.2 对辐射灭菌和消毒单位要求
4.2.1 辐射灭菌和消毒单位的辐射设施安装、鉴定、运行、人员应达到JJG591、GB18871及GB18280要求,同时应取得辐射安全许可证和国家有资质的部门颁发的辐射加工计量许可证;操作人员应经过培训,持有上岗技术考核合格证。
4.2.2 辐射灭菌和消毒单位应有足够的检测能力,能够根据待辐射灭菌产品的生物负载及无菌检测,建立并验证灭菌剂量,保证辐射加工的有效性。
4.2.3 辐射灭菌和消毒单位对于异常情况应有相应的纠正措施和预防措施。
4.3 建立灭菌剂量按照GB18280的要求进行。
产品辐射处理要求
5.1 吸收剂量测量
5.1.1 辐射灭菌单位所用的剂量计应定期校准。
5.1.2 工作剂量计应放在预先确定的常规剂量点,辐射后,测量剂量,记录结果并分析。
5.2 产品的辐射灭菌与消毒
5.2.1 将产品包装的尺寸、密度以及产品在包装内的分布、产品在辐射容器中的装载模式做详细说明。
5.2.2 每个待辐射的都应该做剂量分布图,确定最大剂量和最小剂量的位置和大小,确定最大剂量与最小剂量和常规剂量点处剂量的关系。
5.2.3 待辐射产品应尽量均匀填满辐照容器,其吸收剂量的不均匀度应小于1.5。
微生物监测方法和要求
6.1 初始污染菌检测
每生产批次产品中至少随机抽取10个样品,按照GB18280的要求进行初始污染菌检测。
6.2 消毒灭菌效果检验
每批产品应做消毒或灭菌效果监测。于最小剂量处,每次至少布放10片生物指示剂。辐射后取出指示菌片按《中华人民共和国药典(二部)》(2010年版)的要求进行无菌检查。
辐射产品的放行要求
7.1 产品辐射处理后,对包装完好无损,剂量监测结果符合辐射工艺要求的产品给予放行;对于剂量监测结果合格的符合要求的包装有破损的产品,仅对被损产品包装重新辐射处理,其他包装完好的产品给予放行;对于剂量监测结果不符合辐射工艺要求的产品,应全部报废或重新进行辐射处理。
7.2 放行前,每批产品应出具辐照加工证书。
医疗卫生用品辐射后的管理要求
8.1 辐射灭菌(消毒)后的医疗卫生用品包装箱上应贴辐射化学指示卡、灭菌(消毒)合格证,合格证上应有批号、灭菌消毒日期、有效期、灭菌消毒单位等。
8.2 辐射灭菌(消毒)后的产品应附有辐射灭菌(消毒)工艺的清单,由灭菌(消毒)操作者、审核者签名及质量管理人员审核,其产品的编码、生产批号和产品数量应和入库记载一致。
8.3 待辐射灭菌(消毒)产品贮存库和已灭菌产品的贮存库要严格分开。
8.4 产品辐射处理后,在运输和贮存过程中,如有包装和密封受损破坏,应做报废处理。
附录
附录A(规范性附录):初始污染菌检测
A.1 试剂、器材
A.1.1 洗脱液:含0.1%吐温80、1%蛋白胨的生理盐水。
A.1.2 培养基:营养琼脂培养基。
A.1.3 滤膜(孔径0.45μm)。
A.2 采样方法
A.2.1 对可用破坏性方法取样的医疗用品,如输液(血)器、注射器、注射针、透析器及各类管材等,按《中华人民共和国药典(二部)》(2010年版)规定执行。
A.2.2 对不能用破坏性方法取样的特殊医疗卫生用品要用无菌生理盐水的棉拭子涂抹采样,被采表面小于100cm2 取全部表面,被采表面大于等于100cm2 取100cm2。
A.2.3 敷料类可用无菌操作取10g放入100mL无菌生理盐水中,充分振荡后取样。
A.2.4 采样数量:各类产品每批次随机抽取10件样品。
A.3 检测方法
A.3.1 平板倾注法:分别取1mL处理的洗脱液,接种5个平皿,倾注融化后45℃的营养琼脂,35℃±2℃培养48h,同时作平行样及空白对照。
A.3.2 膜过滤法:对于微生物浓度较低的洗脱液,可用膜过滤法使洗脱液通过0.45μm 滤膜,将滤膜贴在营养琼脂表面培养35℃±2℃培养48h。
A.4 结果计算
A.4.1 平板倾注法,计算见式(A.1): 菌数(CFU/件或CFU/g)=平均菌数× 稀释倍数
